Sport Physicals Announcements

  • Prior to participating on a team in the Lockport City School District, all athletes must provide the Athletic Department with their current address, emergency contact, medical alert and health history information. To expedite this process our Athletic Trainer uses an online data entry system. Below are the directions on how to use the system.     A parent/guardian must join and complete the paperwork. This will need to be done on a computer or tablet. The system is not compatible with a cell phone. 

    You must print the completed Medical History Questionnaire from SportsWareOnLine and bring it with you to your physical appointment.

    Sport sign ups are done through SportsWareOnLine. 

    Sport 1 - Fall

    Sport 2 - Winter

    Sport 3 - Spring  

    Helpful Tips: Highlighted areas need to be completed and if the game/practice status is blank the student will need a sports physical.

Sport Physicals

  • All sport physicals will be performed by the staff of physicians and nurse practitioners from the University Orthopaedics and Sports Medicine office.  You will need to have a sport physical in order to try out for any sport. If a student needs a sport physical but is unable to have the physical on the available dates, they will need to go to UB Sports Medicine. To obtain a physical appointment, please call 716-204-3200. There will be a charge for the sport physical. Please note: Lockport sport physicals are given before each sports season free of charge. 

    If going to UB Sports Medicine to receive a physical you must print this NYS Health Exam Form and the completed Medical History Questionnaire from SportsWareOnLine.
