Effective immediately, sports physicals are no longer required to be completed through UBMD. Any student-athlete who currently has a valid UBMD sports physical on file from this spring will have that physical accepted through the conclusion of the Winter sports season.

Moving forward, all sports physicals must be completed by the student-athlete's Primary Care Physician (PCP) and uploaded directly to their FinalForms account. To upload, please login to your FinalForms account, click the file folder icon, and then click "upload". You can find Final Forms instructions here.

Once uploaded, physicals will be reviewed by the school nursing staff based on New York State athletic participation requirements. Parents and student-athletes should monitor their FinalForms account to ensure all requirements have been completed and approved.

Please remember that sports physicals are valid for one calendar year from the date of the examination.

We encourage families to schedule appointments with their Primary Care Physician well in advance to avoid any delays in athletic participation.
