Dear Parents and Student-Athletes,

As we prepare for the upcoming Fall sports season, please ensure that you have registered for your sport and completed all required forms in your FinalForms account.

Note Important Updates/Changes this year:

Physicals

As stated in our June 24, 2026 email, sports physicals are no longer required to be completed through UBMD. Any student-athlete who currently has a valid UBMD sports physical on file will have that physical accepted through the conclusion of the Winter sports season.

Moving forward, all sports physicals must be completed by the student-athlete's Primary Care Physician (PCP) and uploaded directly to their FinalForms account. Please note that physicals obtained through WellNow, Urgent Care facilities, walk-in clinics, or similar providers will not be accepted.

If you do not currently have an approved physical uploaded in FinalForms, please schedule an appointment with your Primary Care Physician as soon as possible. Remember, physicals must be uploaded to FinalForms for review and approval by the school nursing staff. All forms MUST be completed & signed in Final Forms. 

Please note the following important deadlines:

  • JV and Varsity Football student-athletes must have their physical uploaded to FinalForms no later than Thursday, August 13.

  • All other Fall sport student-athletes must have their physical uploaded to FinalForms no later than Thursday, August 20.

Physicals uploaded after these deadlines are not guaranteed to be reviewed and approved prior to the start of practice or tryouts on Monday. Failure to meet these deadlines may impact a student-athlete's eligibility to participate.

Please plan accordingly and complete all registration requirements as soon as possible to ensure a smooth start to the Fall season.

Advanced Placement Process (APP) for 7th & 8th graders looking to play at the high school level or "play up":

As of July 29, 2026, the New York State Education Department has made the following changes to the Advanced Placement Process (APP) for students in 7th and 8th grade who wish to try out for a Freshman, Junior Varsity (JV), or Varsity sport:

*Please note that these changes only apply to 7th & 8th grade students seeking to participate at the high school level. If your child will be participating in a modified sports program, these new requirements do not apply.

  • The physical fitness test has been eliminated and is no longer part of the Advanced Placement Process.

  • The Tanner Score recommendation has been eliminated.

  • The school district must recommend a student-athlete for participation through the Advanced Placement Process.

The new process:

  • Parent consent and a school invitation are required to participate in the Advanced Placement Process or to "play up"

  • Beginning this season, any parents who believe their child may qualify to play up from the modified level/APP must complete the APP Parental Request for Invitation Form.

  • After a parent completes the Request Form, the student is added to the JV/Varsity roster on Final Forms by the Athletic Office, and are cleared to try out for that sport.

  • Please note: Passing the APP test does not guarantee your child a spot on an upper level sports team, it merely allows them to participate in tryouts.
